HHS Education and Training Curriculum on Multiple Chronic Conditions

The HHS Education and Training Curriculum on Multiple Chronic Conditions (MCC) is designed for educators across the 
educational continuum to equip healthcare professionals and paraprofessionals with tools and knowledge on caring for persons living with multiple chronic conditions (PLWMCC) across settings. The curriculum is based on the Multiple Chronic Conditions: A Framework for Education and Training, which consists of six core domains essential for the interprofessional healthcare team to care for PLWMCC.

Curriculum overview

The HHS Education and Training Curriculum on Multiple Chronic Conditions consists of six modules, Each module contains links to tools and resources that will help enhance the subject matter presented in each module. The introduction PowerPoint presentation provides a general description of multiple chronic conditions and data on dyads and triads of chronic conditions.

The six modules can be used integrated into existing educational programs as a series or separately:

  • Module 1: Person- and Family-Centered Care describes the critical role persons living with MCC play in their care.
  • Module 2: Self-Management Support outlines self-management and self-management support strategies to use to empower PLWMCC. 
  • Module 3: Knowledge for Complex Care describes the acquisition and use of knowledge for practice in caring for PLWMCC.
  • Module 4: Interprofessional Collaboration provides effective strategies for developing and maintaining teams. 
  • Module 5: Coordinated Care Delivery identifies effective strategies and models of care for providing care across settings to prevent fragmented care.
  • Module 6: Systems Based Practice presents key elements that improve the overall health system care of PLWMCC.

Each curriculum module includes:

  • Definition – explains the meaning of the domain.
  • Making the Case – describes the domain’s relevance to caring for PLWMCC.
  • Learning objectives by competency – provides example learning objectives that can be tailored for a specific health discipline or for a specific phase of education.
  • Selected curricular resources – listed from the Education and Training Repository support the domain. Resources were excluded: if materials a) focused on programs, b) focused on treating single chronic conditions, and c) did not address multiple chronic conditions and/or related competencies and domains.
  • PowerPoint presentation – located in the upper right corner of each module. The PPT can be saved, modified and integrated into existing/new presentations.
