Revisions to Certified Provider/Supplier Model Letters and Instructions for Processing Initial Skilled Nursing Facility (SNF) Enrollment Applications
The purpose of this Change Request (CR) is to---(1) Revise the model letters that contractors use when processing certified provider and certified supplier enrollment applications; and (2) Instruct contractors on the processing of initial SNF Form CMS-855A enrollment applications.
