Where do I file a civil rights complaint?

If you are filing a discrimination complaint against one of the following:

  • the U.S. Department of Health and Human Services (HHS);
  • a state or local social or health services agency;
  • a private social service agency receiving federal financial assistance from HHS;
  • a hospital, clinic, nursing home or other agency receiving federal financial assistance from HHS; or
  • a health insurance Marketplace established under the Affordable Care Act,

you can file your complaint via the OCR Complaint Portal, at [email protected], or you can mail or fax your complaint. See Filing a Complaint for more information.
