Goal 1: Strengthen the Adult Immunization Infrastructure


 

The adult immunization infrastructure in the United States is complex and multifaceted, consisting of numerous components with unique functions.This goal of the National Adult Immunization Plan (NAIP) focuses on high-level issues with the potential to have significant impact on adult vaccination rates in the next several years, by improving and leveraging elements that already exist, rather than creating new systems, programs, and entities.

Goal 1 Objectives

The following six objectives, and a number of sub-objectives detailed further in the NAIP, were developed to strengthen the adult immunization infrastructure. The fourth and fifth objectives, in bold below, were chosen in the National Adult Immunization Plan: A Path to Implementation as two of eight implementation priorities.

  • Monitor and report trends in adult vaccine-preventable disease levels and vaccination coverage data for all recommended vaccines.  
  • Enhance current vaccine safety monitoring systems and develop new methods to accurately and more rapidly assess vaccine safety and effectiveness in adult subpopulations, like pregnant women.
  • Continue to analyze claims filed as part of the National Vaccine Injury Compensation Program (VICP) to assess whether there was an association between vaccines that a claimant received and adverse events experienced.
  • Increase the use of electronic health records (EHRs) and immunization information systems (IIS) to collect and track adult immunization data.
  • Evaluate and advance targeted quality improvement initiatives.
  • Generate and disseminate evidence about the health and economic impact of adult immunization, including potential diseases averted and cost-effectiveness with the use of current vaccines.

Goal 1 Priority Measures

The implementation of this plan demands regular monitoring and documentation of progress, challenges, and opportunities. Therefore, the following baseline indicators were selected to measure progress in the NAIP and will inform future implementation and quality improvement efforts:

  • Percentage of adults vaccinated against seasonal influenza annually
  • Percentage of health care personnel vaccinated against seasonal influenza annually
  • Percentage of pregnant women vaccinated against seasonal influenza annually 
  • Percentage of adults aged 65 and older vaccinated against seasonal influenza annually 
  • Percentage of non-institutionalized adults aged 18-64 pregnant women vaccinated against seasonal influenza annually 
  • Percentage of non-institutionalized adults in long-term care or nursing homes vaccinated against seasonal influenza annually 
  • Percentage of adults aged 60 and older vaccinated against zoster 
  • Percentage of health care personnel aged 19 and older vaccinated against hepatitis B
  • Percentage of internists and family physicians who recorded information on adult vaccinations in state and regional immunization information systems
  • Percentage of survey pharmacists who submitted adult vaccination data to an immunization information system
  • Percentage of adults aged 19 and older who had one or more immunizations recorded in an immunization information system
  • Percentage of adult health care providers who identified an adverse event following immunizations that reported one or more events to the Vaccine Adverse Event Reporting System (VAERS)
