7. General formatting for pleadings

  1. Caption. 
    Every pleading and paper filed in the proceeding must contain a caption setting forth the title of the action and the case number.
  2. Title or designation. 
    Every pleading must include a title or designation of the paper, such as “Motion to Quash Subpoena.”
  3.  Signature. 
    Every pleading and paper must be signed by and must contain the address, telephone number, and e-mail address of the party or the person on whose behalf the paper was filed, or his or her representative.  See CRDP § 6(a)(vii) for the signature requirements of electronically-filed pleadings and other documents.
