Assessing the Costs and Benefits of Extending Coverage of Immunosuppressive Drugs Under Medicare
Given the costs to Medicare associated with patients who stop taking immunosuppressive drugs due to cost and revert to developing ESRD, requiring subsequent dialysis and/or kidney transplantation, the Assistant Secretary for Planning and Evaluation analyzed the financial implications of extending Medicare coverage of only immunosuppressive drugs for the lifetime of the patient.

Comprehensive ESRD Care Model
The Comprehensive ESRD Care (CEC) Model is designed to identify, test, and evaluate new ways to improve care for Medicare beneficiaries with End-Stage Renal Disease (ESRD). Through the CEC Model, CMS will partner with health care providers and suppliers to test the effectiveness of a new payment and service delivery model in providing beneficiaries with person-centered, high-quality care.

End Stage Renal Disease (ESRD) Quality Measures
The University of Michigan Kidney and Epidemiology Cost Center (UM-KECC), develops, maintains, and updates the End Stage Renal Disease (ESRD) Quality Measures for the Centers for Medicare and Medicaid Services (CMS), under the Quality Measure Development and Maintenance contract with CMS. These include the Dialysis Facility Compare (DFC) Measures and the development of new quality measures that will be implemented in the future.

ESRD Network Organizations FAQ
An ESRD Network is defined in the Code of Federal Regulations (CFR), at 42 CFR Part 405, Subpart U as all Medicare-approved ESRD facilities in a designated geographic area specified by CMS. These regulations require ESRD treatment facilities to be organized into groups called Networks in order to promote a system of effective coordination.

ESRD Prospective Payment System (PPS) Information
The ESRD PPS provides a patient-level and facility-level adjusted per treatment (dialysis) payment to ESRD facilities for renal dialysis services provided in an ESRD facility or in a beneficiary’s home. The bundled per treatment payment includes drugs, laboratory services, supplies and capital-related costs related to furnishing maintenance dialysis.

ESRD Treatment Choices Model
CMS is proposing the End-Stage Renal Disease (ESRD) Treatment Choices (ETC) Model to encourage greater use of home dialysis and kidney transplants for Medicare beneficiaries with ESRD, while reducing Medicare expenditures and preserving or enhancing the quality of care furnished to beneficiaries with ESRD.

Voluntary Kidney Models
Kidney Care Choices (KCC) builds upon the existing Comprehensive End Stage Renal Disease (ESRD) Care (CEC) Model structure – in which dialysis facilities, nephrologists, and other health care providers form ESRD-focused accountable care organizations to manage care for beneficiaries with ESRD – by adding strong financial incentives for health care providers to manage the care for Medicare beneficiaries with chronic kidney disease (CKD) stages 4 and 5 and ESRD, to delay the onset of dialysis and to incentivize kidney transplantation.

Clinical Trials in Organ Transplantation (CTOT)
The Clinical Trials in Organ Transplantation (CTOT) project is a cooperative research program sponsored by the National Institute of Allergy and Infectious Diseases (NIAID). CTOT is an investigative consortium for conducting clinical and associated mechanistic studies that will lead to improved outcomes for transplant recipients. The purpose of these studies is to improve short and long-term graft and patient survival.

Immune Tolerance Network
For people who receive an organ transplant, immunosuppressive drugs are a lifeline that prevents their immune system from rejecting the transplant. However, these drugs work by suppressing all of the immune system - even the parts that protect from real threats. This means that people taking immunosuppressive drugs are at risk of developing serious infections. The Immune Tolerance Network (ITN) is working to find new therapies that reprogram the immune system in a way that will allow people to maintain the health of the organ without long-term use of immunosuppression, also known as tolerance.
