Terms and Conditions

Recipients of Provider Relief Fund payments and those reimbursed for services by the HRSA COVID-19 Uninsured Program and the Coverage Assistance Fund, must agree to the Terms and Conditions specific to the distribution in which they received a payment or reimbursement.

Providers must attest to program Terms and Conditions in order to receive payment, such as:

  • Eligible providers may use funds to prevent, prepare for, and respond to coronavirus, and for related expenses or lost revenues attributable to coronavirus.
  • Funding cannot be used where another source has reimbursed or is obligated to reimburse those expenses or losses.
  • Payments can be pre-payments, prospective, or retrospective payments.
  • Provider and payment information for entities that have attested to at least one payment is posted to an HHS Public Use File (PUF).

General Distribution

Phase 1

Relief Fund Payment from $30 Billion General Distribution (PDF - 94 KB)
The recipient automatically received payment from the initial $30 billion general distribution.

Relief Fund Payment $20 Billion General Distribution (PDF - 87 KB)
The recipient has received payment from the additional $20 billion general distribution.

Phase 2

The recipients have received a payment appropriated as part of Phase 2 General Distribution, including Medicaid, CHIP, dental, and other providers.

Phase 3

Allocation for Uninsured Patients

Uninsured Program Fund Terms and Conditions (PDF - 124 KB) (For enrolled providers who attested to Uninsured Program funds on or after December 17, 2021)
The recipient plans to submit claims for reimbursement for COVID-19 testing and/or testing related items, care or treatment related to positive diagnoses of COVID-19, and/or COVID-19 vaccine administration provided to individuals who did not have any health care coverage at the time the services were provided. This includes COVID-19 vaccine administration for foreign individuals.

Previous Uninsured Program Fund Terms and Conditions

FFCRA Relief Fund Payment Terms and Conditions (PDF - 529 KB) (For enrolled providers who attested to Uninsured Program funds between February 1, 2020 and May 31, 2021)
The recipient submitted claims for reimbursement for COVID-19 testing and/or testing related items and services provided to FFCRA (Families First Coronavirus Response Act) Uninsured Individuals.

The recipient submitted claims for reimbursement for care or treatment related to positive diagnoses of COVID-19 and/or COVID-19 vaccine administration provided to individuals who did not have any health care coverage at the time the services were provided.

The recipient plans to submit claims for reimbursement for COVID-19 testing and/or testing related items, care or treatment related to positive diagnoses of COVID-19 and/or COVID-19 vaccine administration provided to individuals who did not have any health care coverage at the time the services were provided.

Allocation for Underinsured Patients

COVID-19 Coverage Assistance Fund Payment Terms and Conditions (PDF - 209 KB)
The recipient plans to submit claims for reimbursement for administering COVID-19 vaccines to underinsured patients (defined as those enrolled in health plans that either do not cover vaccination fees or cover them with patient cost-sharing).
