    This Guide describes how rural hospitals, community health centers, local public health departments, and other rural stakeholders can work together to assess and address their rural communities’ health needs. Two innovative rural health organizations that have developed strategies to meet population health goals, addressed social determinants of health, and improved health outcomes are highlighted.

    HHS and HRSA convened a forum of rural health care stakeholders to discuss challenges rural communities face in providing and accessing health care and how HRSA and HHS can address these challenges in the course of program and policy development.
  • Rural Health Information Hub (RHIhub)(link is external)
    Formerly called the Rural Assistance Center, RHIhub is the nation's rural health information source. The website offers a library of resources, coverage of rural issues, state guides, toolkits, program models and more. Customized assistance is available by phone or email. The RHIhub is funded by the Federal Office of Rural Health Policy and all services are provided free of charge.
  • Rural Community Health Gateway(link is external)
    The Gateway can help build effective community health programs and improve services. Resources and examples in this Gateway are chosen for effectiveness and adaptability and are drawn from programs with a strong history of service and community success. Evidence-based toolkits include literature reviews and provide resources to implement programs on topics such as: care coordination, community health workers, mental health and substance abuse, obesity prevention, etc.

    The Rural Health Research Gateway is an online library of research and expertise. It’s free to use, searchable, and provides access to the work of all ten federally-funded Rural Health Research Centers and Policy Analysis Initiatives.

    The FORHP Resources Guide is a non-comprehensive compendium that provides new applicants and grantees with an array of relevant resources, tools and services organized by topic area that will assist in the implementation and sustainability of rural health projects, organizations and networks.
  • Rural Health Grant Eligibility Analyzer
    Health care providers who are located in geographic areas that are defined as Rural are eligible to apply for Rural Health Grants. Use this site to enter an address or choose a state and county to check for eligibility status.
